MUST BE CLEARED IN AKTIVATE TO TRYOUT: Click here to access.
Clearance paperwork must be uploaded into Aktivate and APPROVED by BJHS. (physical must be dated after 3/1/25)
IMPORTANT: In order to ensure that your student is cleared to participate in CUSD athletics, please use the link to the left for Aktivate to complete the clearance process if you have not already for the 2025-2026 school year. Students who have not completed 100% of the clearance process will not be allowed to participate in tryouts until it is completed. Please make sure that you take the required CUSD physical paperwork to your student's physical since the doctor is required to complete one form and sign the other (FORMS 15.7A and 15.7B). All forms are available below.

It is strongly recommended that all documents be submitted electronically PRIOR to the break that precedes your child's sports season. You will be able to register for EACH sport they are interested in playing as part of the registration process.
(The AIA Physical forms are also available for printing at the end of the "Aktivate" registration process)
Physicals must be done on or after 3/1/25. Click the link below for the packet, but it must be uploaded to Aktivate.

$25 per sport / per student is assessed when the athlete is added to the team roster
